About Us

Community Identity Care Services (CIC) is a community-based outpatient mental health center dedicated to providing comprehensive, high-quality behavioral health services to individuals, families, and groups. Founded in 2025 by Pastor Dr. Johnathan Johnson Sr., the organization is committed to promoting emotional wellness, psychological stability, and overall quality of life through accessible, person-centered, and culturally responsive care. Services are delivered in a safe, confidential, and supportive environment guided by trauma-informed and recovery-oriented principles.
CIC provides comprehensive mental health assessments and evaluations designed to identify emotional, psychological, behavioral, and social needs. These assessments serve as the foundation for individualized treatment planning and are updated regularly to reflect client progress and changing needs. Licensed and qualified mental health professionals utilize evidence-based screening tools and clinical interviews to ensure accurate diagnosis and appropriate levels of care.
Individual psychotherapy and counseling services address a wide range of mental health concerns, including anxiety, depression, trauma-related conditions, stress disorders, mood disorders, and life adjustment challenges. Therapeutic interventions are evidence-based and tailored to meet each client’s unique goals, strengths, and circumstances. Services emphasize emotional regulation, coping skills development, symptom reduction, and improved daily functioning.
Group therapy and psychoeducational services create structured opportunities for clients to build skills, enhance interpersonal relationships, and receive peer support. Group sessions focus on stress management, emotional awareness, communication skills, and healthy coping strategies. These services promote social connection, reduce isolation, and reinforce recovery and resilience.
Family and couples counseling services are designed to strengthen family systems and support networks by improving communication, resolving conflict, and fostering healthy relationships. Therapists work collaboratively with families and couples to address relational challenges, enhance understanding, and promote long-term stability within the home environment.
Community Identity Care Services also provides crisis intervention and stabilization support for individuals experiencing acute emotional distress. These short-term, solution-focused interventions aim to reduce immediate risk, de-escalate crises, and connect clients with appropriate ongoing treatment and community resources.
In addition, CIC offers case management and care coordination services to support clients in navigating complex systems of care. Case managers assist with accessing community resources, social services, housing support, employment assistance, and coordination with healthcare providers to address social determinants of mental health and promote continuity of care.
Behavioral health education and preventive services are integrated throughout all programs, focusing on mental health awareness, stigma reduction, early intervention, and the development of healthy coping practices. Through a holistic and collaborative approach, Community Identity Care Services remains committed to empowering individuals and strengthening communities through effective and compassionate mental health care.
Onsite Lab Services
Community Identity Care Services also provides onsite laboratory services through FAIS Lab to support preventive health and wellness needs. Available services include routine blood work, STD testing, A1C and glucose screening, lipid panels, urine testing, and additional diagnostic screenings. These services are offered in a confidential, professional, and client-centered environment to promote accessible healthcare and early detection.
CPR, AED & First Aid Training Classes
CIC is also a certified CPR, AED, and First Aid Training Site dedicated to equipping community members, parents, youth, professionals, and organizations with life-saving emergency response skills. Training courses are designed to promote preparedness, safety awareness, and rapid response during medical emergencies. Classes are hosted by Nurse Brandie Terry, BSN, and provide hands-on instruction in CPR techniques, AED usage, choking response, and basic first aid care in a supportive learning environment.
Health & Wellness Program
Community Identity Care Services offers a Health & Wellness Program focused on improving overall physical, emotional, and mental well-being within the community. The program promotes healthy lifestyle habits, stress reduction, self-care practices, nutritional awareness, fitness education, and preventive health support. Services are designed to empower individuals to achieve balance, wellness, and healthier daily living through education, support, and community engagement.
Yoga & Mindfulness Classes
CIC also provides Yoga and Mindfulness Classes designed to support relaxation, emotional balance, stress management, and overall wellness. These classes encourage healthy breathing techniques, mindfulness practices, flexibility, and physical activity in a calming and supportive environment. Yoga sessions are structured to promote mental clarity, emotional resilience, and holistic wellness for individuals of all ages and experience levels.
Summer Camp Program for Children & Youth
Community Identity Care Services proudly offers a Summer Camp Program for children and youth focused on enrichment, wellness, education, recreation, and social-emotional development. The program provides a safe, structured, and engaging environment where youth can participate in educational activities, team-building experiences, health and wellness programming, mentorship, arts and recreation, and life-skills development. The summer camp program is designed to empower youth, encourage positive peer interaction, and support healthy growth within the community.

Our Founder & CEO

Dr. Johnathan M. Johnson Sr. is the Founder, President, and Chief Executive Officer of HB Community Identity Care Services, bringing over ten years of experience in the mental health field. His commitment to community organizing and empowering both youth and adults is deeply rooted in his personal life experiences and a genuine passion for creating positive, lasting change.
Based in Illinois, Dr. Johnson leads his organization with a vision that extends beyond local impact to global outreach, striving to provide essential mental health and supportive services to individuals and communities in need. His work emphasizes access, empowerment, and holistic care, addressing emotional, medical and social well-being.
Dr. Johnson’s impact and service have been recognized through numerous publications and honors that acknowledge his ongoing contributions to communities across the region. His work has been featured by organizations and media outlets including the NAACP – East Chicago Branch, Chicago Defender Newspaper, Chicago Tribune, Northwest Indiana Times, WGN News, CBS News, FOX News, Harvey World Herald, and others. He has also been recognized on multiple occasions as a United States of America Honorable Leader Volunteer Awardee.

Dr. Johnson previously served as Chairman of the Chicago Baptist Institute’s Advisory Board and currently holds the position of Vice President of Living Waters Counseling and Consultations, further reflecting his commitment to leadership, counseling, and community development.
